Repeat
Prescriptions:
There are several options to obtain your repeat
prescription:

Prescription request form (right
hand side of prescription)

You can use the
online prescription request
service which will email your
request directly to the receptionist. On this form
there is the option to have your prescription sent back to
the college nurse - please make sure you indicate this on
the request

In writing with a stamped
addressed envelope.

register:
Please print off the registration
forms using the link for the appropriate college below
and send them back to Summertown Health Centre, 160
Banbury Road, Oxford, OX2 7BS, UK when completed.
Please note, your home address should be your college
address.
You need to
complete firstly the
GMS1
followed by the questionnaire for your college.
